As for the most realistic scream of fear, I'm torn between two options:

"Psycho" (1960) https://www.imdb.com/title/tt0054215/ : Marion Crane's iconic shower scream is in movie history. The wide-open mouth, contorted eyes, and windblown hair convey pure terror and vulnerability. It's not just a scream; it's a snapshot of a soul being ripped apart.

"The Shining" (1980) https://www.imdb.com/title/tt0081505/: Wendy Torrance's scream through the cracked doorway in Room 237 is equally chilling. It's not a full-blown roar, but a choked, desperate gasp that speaks volumes about the mounting horror she's facing. It's the scream of someone on the edge, barely clinging to sanity.

It is supossed that are the mos realistic screams on screen.
