Can’t delete my page

I helped a friend with an iphone made short and did not realize she was going to upload it to IMDB. She created a credit and a page for me without my permission. I am a crisis counselor and do not want this appearing when clients search for me.  I have tried three times to request deletion and it has not been accepted - and i don’t know why. I’ve uploaded my ID to verify that i am this person. What do i need to do?
