What about actors that are deceased over twenty years? Is there a way to add a photo, or to request that a photo be added, or to request that the primary photo be changed?
There are some wonderful photos of some actors on the title pages, but they are not on the name pages for the actors. Would be wonderful if there was a way to request that some of the best ones to be on the actors’ name pages.
Not even for deceased actors. Currently the only way to change the profile picture is to be a IMDbPro member and have your own page claimed and that only makes you available to change your own picture.

You'd have to be the executor of the deceased's estate (which would be next of kin by default) or one of their existing talent representatives, if you wanted to be able to manage the featured images and other IMDbPro features. If you have neither a need nor a desire to be able to control the content of an IMDb page on a whim, then you might be alright to drop by the forum and suggest that a specific page have a specific image having a relaxed distribution license as the perpetual primary image placeholder. The IMDb staff won't be able to assist with such requests for the foreseeable future, but at least there will be a record of the recommendation for future reference.
