In this thread, you asked me to report any further instances of this happening.

I am unable to respond in that thread because it has been closed by a member of staff, but I have come across another occurrence, for which I have submitted a correction - 221026-080507-551000

This is the only one I have found since that thread was closed, so admittedly it seems to be occurring much less frequently than before.
