When an episode title has an image, but not a default image
Fox News Copes & Seethes Over Trump Debate Blowout
I see this happen once in awhile: an episode title has an image, but not a default image. What causes this, and can I fix it myself? Because I've seen other examples.
Notice that in this example, the episode's one image does not appear to the left of the title, as normal—not on the episode page, and not on the episode list.

In non-Reference view, the "facts are stubborn" image appears on the title page for the episode. But when I click on it, I get the Trump photo, which is 1 of 1.
So the link for the episode poster leads to the episode's images, not to an enlargement of the image itself? Not what I would expect.
When I look at the series level, the "facts are stubborn" image is the Poster for the title:
