What's wrong with this goof?

Below is a goof for the film Tremors. It keeps getting declined. Can anyone tell me what's wrong with it? I'm baffled as to why a goof like "Walter nose is bleeding before he gets gabbed by the tremor." can make the cut but mine was declined. Goof Addition (Continuity) When they are all standing around watching Burt remove the Graboid tentacle from Val and Earl's truck there are duplicate shots, though not the same exact takes. The shot reused is the one where Val says "I didn't know we had it." During the shot Heather walks from behind Val and Earl to the other side of Burt, while Nestor walks from the left side of frame to the middle. The second time the shot is used is right after Burt says "Relax. It's dead." After his line both Nestor and Heather are back where they were, and Val can be seen saying his "I didn't know we had it" line again but with no audio. In the first take used Heather continues looking at the truck when Val says the line. In the second take she looks at Val when he says the line.
