What is going on here? (Weird name pages with URL code for name)

When you open up one of the above two pages, you are taken to the name page of a crew member with the name "nm0999242", which happens to be IMDb's URL code for an obscure actress named Cláudia Lima.
I've been noticing this happening a bunch lately; when creating lists, names pop up with IMDb URL code things instead of, you know, an actual name. These two popped up when adding David Wood to a list.
EDIT: There are way, way, way more examples of this on IMDb than these two that I just brought up, by the way. I see it all over the place.

I submitted name merges with the original Cláudia Lima page. (Contribution #230129-210221-261000 & Contribution #230129-210333-599000) The most plausible explanation for this is that credits belonging to Cláudia Lima were mistakenly added as a "new name" using her URL code. So that part is taken care of, for now (assuming the merges get approved).
However, I don't think this thread should be marked as solved until someone answers the question of (a) why this is so rampant on the site and (b) how can we find and correct ALL instances of this on the site, not just the ones I happened to find today and (c) what safeguards can be put in place to prevent this from happening in the future.

Case in point: I went back to the list I was making earlier and already I have found two more examples of this problem.

These ones bear the name "nm14184402", the URL code for Andy Elias, who appears to be some kind of porn star. These popped up when adding the name Ivana Baquero to the list.
