Reason: Duplicate

Some submissions are labeled "Needs Attention" with this explanation:

"Reason Duplicate

We were unable to approve this contribution.

The item you have submitted is exactly the same as a submission that you have already entered, which is still pending review by our staff ..."

I have seen others mention it too recently. The problem is that occurs on submissions where the description is not true at all.
