The IMDb Parents Guide sections are written by contributors in wiki style, meaning that people can edit them at any time and have their edits show up immediately (as opposed to having to submit their contributions to IMDb staff for consideration). If you don't have access to editing the Parents Guides (access has been limited in order to avoid spam), you can request access by submitting a request at http://www.imdb.com/helpdesk/swikieditapply.
Since the Parents Guides are in wiki style, there is no effective way to enforce a particular format of submissions.
That said, the Parents Guides normally make it clear what gender the participants in the sex scenes are. (I mean, if the Parents Guide has actually been written. There are plenty of titles in the database that don't have Parents Guides at all.) I doubt you will find many Parents Guides where there is a description of a sex scene in a film but it's left ambiguous what gender the participants are. And if you mean that there should be sub-headings under "Sex & Nudity" for Straight, Gay, and Lesbian, few mainstream films have so many sex scenes that one would need categorization to keep track of them.
