Hi, panagiotis1993. In order for the option to "suggest as primary image" to appear in the image tag editing menu, an image has to be categorized as a poster and its width must be shorter than its height, preferably being as close to two thirds of the height as possible. For a title page that has no primary image assigned and no image gallery item eligible for assignment as the primary image, a newly uploaded image will automatically become the primary image (or rather the default profile image) regardless of its width-to-height ratio, as long as it is categorized as a "poster" or a "still", but upon the upload of an image with poster qualifications, the default profile image will become that new image. On a side note, the way primary images for episodes work is different from the way that primary images of movies (and series and video games) work.

Some of the information I provided is documented at https://help.imdb.com/article/contribution/images-videos/how-do-i-update-my-poster/GA8N34CWGUMJJM98#. On account of the width-to-height ratio requirement not being documented in the IMDb Help articles, I'm not entirely certain that it still exists.
