New images not attached to titles

What does it mean when an image is accepted but is not attached to the title?

You can only discover this through IMDb Contribution: Your Contribution History as images are not mentioned in https://contribute.imdb.com/updates/history

If the image is accepted but not attached to a title, then where can you find it on the site to reattach it?  It will not show up on the name pages of the people included, as I think there must be different rules on images depending on who uploads them.

ie some images once mapped to a name appear in that name's photo section, and some even appear as the main image for that person.  I am guessing this hierarchy depends on if an agent or production company first added the image.
