Need to Delete a Credit that was not mine

Have you been credited in the movie? if not, can you help providing a screengrab from the onscreen credits so we can verify. We were unable to verify the deletion.

No I was not credited on the movie - the producer just gave me a Special Thanks and added my name on the IMDB Pro and IMDB credit list under thank you - I NEVER worked on the film and I do not want my name to be on it.
