I can confirm that the visual effects department credit was not approved because our editors were unable to verify the information.  You will need to resubmit the credit with the on-screen evidence (showing your name credited on-screen for the role).  For details on how to provide credit evidence I encourage you to review our Help guide.

Once resubmitted, our editors will take another look.

Hi Michelle, I resubmitted my credit with Screenshot PNG with episode name to verify as per given upload guideline, hoping editors of imdb will look into it soon and add it to my list as its been long pending submission from my end.
Below is my new ref id number as earlier one is declined and closed for the second time. 
I have provided my name mentioned screenshot as upload onto given id .
looking forward to imdb rectifying by not declining again. attaching same image for your reference
