How to change my name?

Hello!

I recently requested a name change and it was rejected, reason "unable to verify." ( #220217-214532-693000)

I unticked preserve name on credits. I did not upload any evidence. 

Can someone offer suggestions on how I can get my name changed? I am just changing my first name. I will never return to my original first name, because it's similar to a new p*rn star. I have recently changed my all my social media handles to my new name as well. All that remains is for IMDB to change it too.
