Onscreen credit is just this:

"Song Name"

By Firstname Lastname

Also, when different artists were not seperated with anything onscreen, should they be seperated with something like "and" or "&" despite they are not on screen?

Hi abcd -

The information you submit should be exactly as its shown in the movie. So if only the song name and the performer are shown on-screen, the credit should be listed as:

"Title of Song"
Performed by person(s)/group

In instances where multiple people are listed, the names should be separated with commas and use of "and" or "&":

"Title of Song"
Written by Person A, Person B, Person C and Person D

The guidelines are unclear because they say "exactly as its shown in the movie" (should be it's, by the way), but also specify formatting that needs to be used. Similarly, in your reply you add the word "performed" even though it was not included on screen in the example, and it is not necessarily clear whether "by X" means that X performed or composed the track.
