How are outtakes or deleted scenes from a movie or tv episode typed?
A deleted scene is one originally part of the movie or tv episode, but cut before release or broadcast. It sometimes gets restored in "directors cuts" and rereleased. Otherwise it often gets released on DVD as a bonus.
An outtake, also known as an alternate take, is one that was never part of the movie or tv episode, though it was shot as intended for it. It sometimes gets released on DVD as a bonus.
These DVD bonus tracks, even on those that may be restored into the full movie/episode and rereleased/rebroadcast, usually have their own separate title entry on IMDb, but would they be typed as (V) for Video, or would the original type be used (TV) for tv episode/tv movie or no type for theatrical movie?

These titles would be considered "Made for Video Movies" and should be formatted as "Made for Video", for example:
Title (year) (V) i.e. Aladdin and the King of Thieves (1996) (V)
Concerning your second question, we do allow separate titles for Deleted scenes, for example see:
