Having problems trying to remove an Image - PLEASE HELP

Hello, 

For past week, we have been trying several times to erase this image. 

For some reason, the submission always gets rejected.

We truly appreciate if you can remove it for us ASAP. There were typos and names written incorrectly. 

In the future, please let us know how exactly we need to type on the "edit tag section" to remove this image, in case some misfortune happens again.
