declined image submission (clarification)

Could I please get some clarification on the declined image submission #260302-144318-996802?

Shows "approved" and then "declined" with the unable to verify notice. The image comes from the Vinegar Syndrome blu-ray of the Jose Benazeraf films. I didn't submit any name for the actress as didn't know that, but am certain the image is for the correct movie as I just watched it. Was this declined in error, or is there something else? I submitted two images at the same time, the other was approved right away.
