Another glitch in the beta-mode Contributor History (Image submissions don't always appear)

Lately, I'll submit an image to the database. Then it will not appear in the beta-mode Contribution History. Nor will I receive an email confirmation.

I submit the image again, and the system immediately rejects it for being a duplicate.

This does not always happen. Most times, the image submission appears in the Contribution History as normal. But for the past few days, this problem will happen at random times.

For example, I just submitted an image to an episode of "The Matt Walsh Show." The submission did not appear in the beta-mode Contribution History, nor did I receive an email confirmation. I submitted the image again, but the system immediately rejected it as a duplicate. See the screenshot below.
