Go to the page for the film and click "Edit page" near the bottom of the page.
On the next page, go to the "Goofs" line (which will be under the "Fun Stuff" heading). On that line, select "Add 1 item". Scroll down and click "Continue".
On the next screen, you will be asked to indicate what type of goof it is (see the explanations of the various categories at https://help.imdb.com/article/contribution/titles/goofs/GFDUF27RBTQGS8UZ#categories ). In the Goof box, enter the description of the goof exactly as it should be displayed on IMDb (with nothing extra). Click "Check these updates". The rest of the submission process should be self-explanatory.
