My former assistant used my corporate credit card to set up an account and we've been billed $21.19 a month for a few years now. I discovered that no one has used the account for quite a while and I can't cancel it because no one can locate the login and password.

I tried that. This is the answer I get, and the problem is I don't have the email address she used:  

I'm Sally from IMDb, I hope you are having an amazing day.

Please add the card that was charged as a new payment method to any Amazon account. Once this is done, please write us back with the email address associated to this Amazon account so we can search our records for  accounts with charges to this card.

@afwalter  You do not need the email address your assistant used.  Please see Sally’s message — we need you to add the card to any Amazon account under your control and then reply to Sally’s message with the email attached to the Amazon account you just added it to.  IMDb is a subsidiary of Amazon and payments for IMDbPro are made via Amazon so we need you to follow these steps in order to locate the account.
