Wrong Date Of Birth - I have submitted proof but it is NOT been corrected.

If that long ago, you're submission was not accepted for some reason.  You might post the submission ID number and see if staff can check it and advise you on what you need to do.  Or just re-submit it, add your proof on the data verification form http://www.imdb.com/helpdesk/data_verification then paste in the new submission ID number and have staff check on it for you.

You could upload your documents to a pic hosting site or use drop box, which would be a more secure way of showing the forms, and linking on the verification form for staff to see them.
