Removing Trailer

Colleen.  I need to be removed from a TAG in a trailer/video I'm not at all part of.  How can I remove my tag from the video YELLOW FEVER? Can you help with this.  It's also attached as a main video on my page and it's from a long time ago and I'm not in the video at all.  Katrina Begin
