She wants to pass on to the page editor, who probably could not verify what you submitted, proof they exist. So, a screen capture, as she described, would be the best proof since that's what IMDb seeks to document. Images can be uploaded here in a reply, using the small camera icon. You mention a "list of changes" but you only provided the submission ID for one change above. If there are more you're asking about, also add those ID numbers and screen capture of those names appearing on screen.
IMDb is primarily interested in the on screen credits. So the evidence they want is a copy of your name in the credits on the screen.
You should get a screencap of that credit proving that you were actually credited as production assistant.
You can upload that picture to a reply here by using the camera icon in the menu under the text entry box.
If your name did not appear on screen, then you will need to resubmit your request, but with an attribute of (uncredited). In that case, contracts, call sheets or pay stubs would be the type of evidence needed.
Is it possible to implement the changes with some urgence? Our composer Michel Banabila is a finalist in a contest, and the change on IMDb is necessary for the jury to validate his role.
